Licensed Practical Nurse (LPN) Home Care & School Nursing (Per Diem)

We are seeking dedicated Licensed Practical Nurses (LPNs) to join our team, providing compassionate care for pediatric and adult clients in home care and 1:1 school nursing settings across Long Island.

What You'll Do:

• Provide direct patient care under RN supervision, including administering medications and treatments.
• Monitor and document patient health status and communicate changes promptly.
• Support clients with activities of daily living while maintaining their comfort and dignity.
• Provide 1:1 care for pediatric clients in school settings, ensuring a safe and healthy environment.
• Collaborate with families and the care team to implement personalized care plans.

Requirements:

• Current New York State LPN license.
• 1+ year of experience in home care/school
• CPR/BLS certification (American Heart Association or Red Cross).
• Physical exam within the past year.
• QuantiFERON within the past year.
• MMR documentation or titers.
